Day Care Inspections
 To apply for a Day Care Inspection the provider must contact the Fire Prevention Bureau at 801-799-4150, 801-799-4154 or by email and submit payment for the inspection. Be sure to provide your Name, Address and Telephone numbers where you can be reached. The fee for inspection is (see fee schedule). This fee is due at the time of the application. |
